The Grind: Chapter 10
The team voted to: Hayley physically shoves Daniel aside to drill the pediatric IO and initiate massive transfusion herself, completely ignoring Mallory's grotesque, futile CPR on Bed 3.
Across the room, the sickening, wet *crunch* of Mallory performing one-handed chest compressions on a corpse fills the air. The dead farmer's neck is grotesquely inflated with subcutaneous air from the misplaced airway. Donna aggressively spikes a second unit of pediatric blood, then turns and grabs Mallory's wrists with iron force. "Time of death, 0722. Stop breaking his ribs, Dr. Knight. Environmental is bagging him now. Walk away." Mallory violently flinches, dropping her hands as if burned, and backs into the corner, her neurodivergent brain redlining from the auditory chaos and the pungent garlic stench of the toxic sludge.
The brief illusion of control shatters as the double doors from the waiting room fly open. Lupe, the unit clerk, and a security guard violently heave a 30-year-old man onto Bed 1. "Triage dumped him in the lobby two hours ago for a 'superficial' chest scratch!" Lupe yells over the din. "He just collapsed in the vending machine aisle!" Hayley pivots. The man is gray, diaphoretic, and gasping. A tiny, half-inch puncture wound sits over his left ventricle. His neck veins are massively distended, bulging against the skin. Beck's triad. "Muffled heart sounds and JVD," Hayley diagnoses instantly. "He's in obstructive shock. His pericardial sac is filling with blood and strangling his heart."
Before Hayley can grab a spinal needle for a bedside pericardiocentesis, Dr. Elena Garza, the General Surgery senior resident, strides into the bay. Garza takes one look at the five-year-old in Bed 2 and shakes her head. "I'm not doing a blind ex-lap on a pediatric auto-ped, Carter. I don't care if the FAST is positive. Package the kid and get him to the CT scanner so I know if I'm cutting spleen or liver." Hayley's jaw clenches. The child's temporized blood pressure will inevitably collapse inside the 'doughnut of death' without immediate surgical source control. But Bed 1 is seconds away from a PEA arrest from the tamponade, and Daniel is paralyzed by his previous failure. Hayley is the only senior doctor capable of fighting Garza or decompressing the heart.

Hayley aggressively blocks the CT scanner route, demanding Garza take Bed 2 straight to the OR, forcing the trembling Daniel to attempt a high-stakes, ultrasound-guided pericardiocentesis on Bed 1 alone.
