The Grind: Chapter 5
📊 Global Attending Consensus
The team voted to: Mallory pushes past her sensory aversion, stepping into the mess to rapidly push massive, escalating doses of Atropine to dry the secretions before anyone attempts an airway, delaying oxygenation but protecting the staff.
"Secretions are clear, but he’s seizing. He needs Versed, two grams of Pralidoxime, and a tube," Mitch notes, already unlocking Bed 2's stretcher. "I’m taking the neck to Dr. Onlychess in the OR before she finds a bureaucratic reason to reject him. Try not to burn the department down while I'm gone." He kicks the brake release and rolls Bed 2 out the double doors. The bay is quiet for exactly four seconds before Dr. Hayley Carter walks in, chart in hand, just as the EMS radio shrieks a new arrival.
The ambulance bay doors crash open. Paramedics sprint a 22-year-old female into Bed 1. "Found syncopal in a dorm bathroom," the medic pants, his boots squeaking loudly on the linoleum. "Heart rate 145, pressure 70 over 40. Abdomen is a literal board." Hayley instinctively grabs the ultrasound probe, slapping cold gel onto the girl's pale belly. The machine beeps, and the screen instantly displays a massive, anechoic black lake of blood in Morison’s pouch and the pelvis. "Positive FAST," Hayley mutters. But as she realizes it's a ruptured ectopic pregnancy, her hands begin to violently shake. The visceral memory of her own miscarriage floods her vision. She drops the probe and physically takes a step back from the bed. "Page Dr. Sterling in OB," she orders, her voice hollow and detached.
"Sterling is a wall. He won't accept her without a formal radiology read," Donna snaps, the mechanical whine of the Belmont rapid infuser spinning up as she spikes O-negative blood. "She doesn't have time for a trip to the CT scanner. She's bleeding to death right now." Tara glares at Hayley's hesitation and grabs a massive 18-gauge needle. "Then we initiate MTP and I'll drop a Zone 3 REBOA to clamp off her pelvic blood supply until Sterling gets off his ass!" At Bed 3, Mallory is struggling to hold the seizing farmer down, desperately needing someone to push the paralytics and secure the airway. Hayley is paralyzed by her trauma. Tara is about to go rogue. Bed 1 is bleeding out, and Bed 3 is brain-damaging. Who takes control?
What are your orders, Doctor?
Hayley violently pushes past her trauma, taking Bed 1 to aggressively deploy the Zone 3 REBOA herself, leaving Mallory to secure the Tox airway alone (explicitly using Rocuronium, as Succinylcholine is fatally contraindicated in organophosphate tox).
Execute Option 1Hayley completely avoids the OB case, pivoting to Bed 3 to secure the airway, giving Tara the green light to recklessly attempt the blind REBOA insertion on Bed 1 without attending supervision.
Execute Option 2Mallory abandons the Tox patient's airway to physically block Tara from the unauthorized REBOA, forcing Hayley to snap out of her paralysis and manually resuscitate the ectopic pregnancy while Bed 3 continues to seize.
