The Grind: Chapter 9
📊 Global Attending Consensus
The team voted to: Hayley ignores the PEA tox patient, executing a flawless pediatric RSI and hypertonic push, forcing Mallory to initiate traumatic CPR and blindly force the bougie through the false lumen herself.
Mallory railroads a 6.0 Shiley tube over the misplaced bougie and attaches the BVM. She squeezes the bag. The farmer's chest does not rise. Instead, the air is forced directly into the mediastinum. His neck and upper chest instantly inflate with a horrifying, Rice-Krispies crackle of massive subcutaneous emphysema. "I'm in the false lumen!" Mallory screams, her neurodivergent brain redlining from the auditory and tactile chaos as she initiates frantic, one-handed chest compressions on the pulseless, toxic corpse. Donna doesn't even look over, mechanically spiking a pediatric unit of O-negative blood. "Bed Three is a balloon animal, doctor. Stop compressing and call it," Donna deadpans, her voice devoid of pity.
But Bed 2 is crashing. The hypertonic saline is shrinking the child's swollen brain, but the auto-pedestrian impact pulverized his internal organs. The monitor shrieks a new, lethal cadence: Heart rate 170, blood pressure plummeting to 45/20. "He's emptying his tank into his belly!" Hayley growls, tearing open a pediatric EZ-IO kit. "Farmboy! I need a tibial IO now so we can pressure-bag this blood, or he codes in thirty seconds!"
Daniel stands frozen at the foot of the bed. He holds the pink pediatric intraosseous drill like a loaded handgun, his massive, trembling hands terrified of shattering the child's fragile, toothpick-sized tibia. "I... I can't palpate the tuberosity!" he stammers, sweat dripping from his nose onto the bloody drapes. At Bed 3, Mallory is hyperventilating, rhythmically crushing the chest of a dead man whose neck is actively expanding with trapped air, refusing to stop the futile code. Hayley has a crashing kid with failing access, a paralyzed intern, and a senior resident having a breakdown over a mutilated corpse. She has seconds to intervene.
What are your orders, Doctor?
Hayley orders Donna to physically restrain Mallory and definitively call the Tox code, forcing Daniel to push through his terror and drill the high-stakes pediatric IO himself.
Execute Option 1Hayley physically shoves Daniel aside to drill the pediatric IO and initiate massive transfusion herself, completely ignoring Mallory's grotesque, futile CPR on Bed 3.
Execute Option 2Hayley temporarily stabilizes Bed 2 with a peripheral push-dose epinephrine bolus, sprinting to Bed 3 to surgically open the neck and find the true trachea to salvage Mallory's patient.
Execute Option 3
