"Delgado, cover the intestines with warm saline drapes and don't let them dry out," Mitch ordered, abandoning the electrical burn in Bed 2. He pivoted to the cyanotic asthmatic in Bed 3. "Ketamine and Rocuronium. The Ketamine will bronchodilate, but if we don't give him a massive expiratory phase, he'll breath-stack and pop a lung." Mitch swept the Mac 4 blade past the swollen cords and dropped a 7.5 tube. "Tube's in. Set the vent rate to eight. I:E ratio one to five. Give him time to exhale."

Across the pod, Hayley and Daniel descended on the crush victim in Bed 1. Dark blood steadily flowed from the calf fasciotomies onto the linoleum. "Blood bank is dry on O-negative," Hayley barked, tearing open a massive hemorrhage kit. "Farmboy, push two grams of TXA and four grams of Fibrinogen concentrate. We have to artificially rebuild his clotting cascade before he bleeds out." Out in the hallway, a psychiatric patient shrieked, violently hurling a plastic urinal against the wall. The crash of shattering plastic barely registered over the blaring monitors.

A brief ten seconds of control settled in. Mitch leaned against the vent, popping his neck. "If I have to look at another open abdomen today, I'm ordering takeout," he muttered. Daniel wiped sweat from his forehead, looking at his blood-stained hands. "I think I'm going to become a radiologist. It's dark and quiet." Hayley snorted, tossing an empty vial into the sharps bin. "They still make you talk to surgeons, Farmboy. Not worth it."

The illusion of peace shattered. Bed 3's ventilator shrieked a high-pressure alarm. Peak airway pressures hit 65 cmH2O. The asthmatic's blood pressure plummeted to 60/40. "Auto-PEEP!" Mitch yelled. "He's breath-stacking! The air is trapped and crushing his heart!" Simultaneously, Bed 1's crush victim started seizing—a massive calcium drop from the aggressive factor replacement and underlying crush syndrome. And at Bed 2, Delgado yelled out, "Dr. Rabinowitz! His core temp is thirty-four degrees and dropping! The wet drapes aren't holding heat, he's going into a hypothermic coagulopathy!"

The ER is redlining. Mitch has to immediately disconnect the asthmatic's vent and manually decompress his chest, Bed 1 needs immediate calcium and Ativan for the seizure, and Bed 2 requires an emergent sterile Bogota bag closure before the hypothermia becomes irreversible.
